ANALISA PEMANTAPAN MUTU INTERNAL PEMERIKSAAN ENZIM HATI BERDASARKAN ATURAN WESTGARD DI LABORATORIUM RUMAH SAKIT

Background: Internal Quality Assurance (PMI) is a prevention activity and supervision that is implemented by continuous laboratory monitoring to prevent or reduce the occurrence of errors/deviations. The scope of internal quality assurance objects includes the analytical stage, namely quality control, which is the stage where precision and accuracy tests are carried out. One of the applications that can be carried out is by using the Levey Jennings chart, then his reading can be used with Westgard rules to assess the accuracy (d%) and precision (CV%) of the examination. Research Objective: This study is to determine the internal quality improvement of liver enzyme examination based on the Westgard rules in hospital laboratories. Research Method: Descriptive Quantitative. This study was conducted at Hospital X and Hospital Y on March 10–16, 2025. The population collected was all secondary data from the quality control results of liver enzyme examinations (AST and ALT). The samples used in this study were secondary data from August–December 2024. Results: Value The accuracy of liver enzyme examination at Hospital X has a good bias value, indicated by no bias value exceeding the maximum limit of ±10%, and Hospital Y has results bias, which is quite good, with 4 months of QC results not exceeding the maximum limit and 1 month of inaccurate QC results because they exceed the maximum limit. The level of precision in liver enzyme examinations at Hospital X and Hospital Y has a fairly accurate level of precision because there are no CV results that exceed the maximum limit of 7%. Evaluation of the Levey-Jennings graph using the Westgard rule on liver enzyme examinations at Hospital X found many 1-2S and 10x rules. While at Hospital Y, I found many 1-2S, 4-1S, and 10x rules. Conclusion: There is a significant difference in the results of the quality assurance research: internal audit of liver enzymes based on the Westgard rule in the hospital laboratory
